The primary function of a scribe is contemporaneous creation of a medical chart, done under the direct supervision of the health care professional. The scribe will document the patient’s history, the health care professional’s interaction with the patient, the procedures performed, the results of laboratory studies, and other pertinent information.

Additional functions may include the ordering of laboratory and radiology studies, documentation of disposition and discharge instructions, documentation of consultations, and notification of completed investigations.

Every minute counts! The average time to complete a chart is 5-10 minutes. The average time it takes to make an EMR entry is 10-15 minutes. If a typical physician or health care professional sees 20 patients per shift, then the time he or she spends on documentation instead of with patients can equal 100-200 minutes!
